When you upload a video do you not get the option on the bottom right to upload a custom thumbnail?

nope. asks if i want to pick a frame from the video or it says "To be able to add custom thumbnails your account needs to be verified and in good standing."

I just went through the verification process (they text you a number, you enter the number, done). I was not aware that was a thing. i guess that should answer my original question.

It's seems fair I remember doing this, it's their way of making sure your a real person, etc. YouTube is getting stricter and stricter.

Yeah, forcing google+ on you is pretty lame...the mobile phone number was a pain for me as well as I dont own a cell. Had to use the wifes.

That cellphone number harvesting is getting annoying. It seems that is not enough for google to own our smartphones.
Microsoft is doing the same, I had to do a chain-verification of all my "hotmail" and "live" email accounts to avoid giving them my number.

Thanks for asking/answering this! I've been wondering about the custom thumbnails. I was so jealous everyone else got to add text and mine all look the same.
