Anyways, better late than never >_> :...

1. Trying to take off your helmet before taking off your glasses ~_~ .

2. Not pulling in the clutch all the way while shifting because of not paying attention ~_~ .

3. Going to take off after parking and clunk - disk locks still on the rotors. Turn off bike, take locks off, put under seat, restart, roll ~_~ .....
Number 1 on your list... All the damn time.

One time years ago I had hair going halfway down my back. Man I miss it... wish the wife wouldn't have made the sacrificial cut with paper shears :( .....
 
The garage exit sensor to open the gate from the underground parking at my work. The sensor works properly for entering the garage on my motorcycle with no fuss. But exiting, most of the time it will not sense it. You need to swipe an RFID card and it needs to sense a vehicle present for it to open and on top of that, it's on an backwards incline. One time last year, I was waiting for almost 20 minutes to get the gate open. I was pissed.

I've also had problems with left turn sensors at intersections not sensing the bikes, but one 3 minute call to the city generally fixes that.
 
1) People moving over without indicator
2) Being too excited to ride my bike that I walk out of the house with all my gear on except my helmet o_O
